Sometimes more than one person can hold a TM for exactly the same name, as long as it is used for a totally different purpose.

Not as far as I know, & I doubt that YouTube would have any legal recourse against GoodTube.com or similar names, unless they are very similar to YouTube itself like YouTueb.com, etc...
